Job Summary
Our client, Disability Network Washtenaw Monroe Livingston, is seeking a committed Community Transition Specialist to assist adults and seniors with disabilities, seamlessly reintegrating them into their communities of choice. Utilizing Motivational Interviewing and Person-Centered Planning techniques, this role aims to empower individuals to achieve independent living.

The ideal candidate needs diverse expertise, including legal systems, education design, and an understanding of human behavior, group dynamics, societal shifts, and cultural backgrounds.

Responsibilities
  • Will be working out of the Ann Arbor office and servicing consumers in Washtenaw, Monroe, and Livingston Counties
  • Coordinate access to community resources for achieving independent living goals
  • Provide coaching and peer support to aid individuals in effectively managing disabilities
  • Educate nursing facility residents and staff about available services through Disability Network Washtenaw Monroe Livingston
  • Facilitate person-centered planning for transitioning individuals
  • Develop comprehensive transition plans including housing assessments, skill training, and service identification
  • Act as a liaison with community agencies involved in the transition process
  • Secure and manage direct transition funds for successful reintegration
  • Offer follow-up support services to individuals’ post-transition
  • Attend relevant training sessions to enhance service quality
  • May be required to perform duties in outdoor environments
  • All other duties as assigned
Requirements
  • Bachelor’s degree in social work, nursing, health or human service-related field, and or a Community Health Worker certificate (required)
  • 3+ years of experience in healthcare or care of individuals (required)
  • Valid Michigan driver’s license and proof of vehicle insurance
  • Personal experience with disability and/or knowledge of disability issues and the consumer/community-based IL philosophy
  • Possesses strong communication skills
  • Must be able to sit for extended periods and lift objects weighing up to 50 pounds

Job Type: Full-time
Pay Rate: $35,000-$40,000
