Our mission is to assist individuals in seamlessly reintegrating into their communities of choice through a person-centered approach, guided by the principles of Motivational Interviewing and Person-Centered Planning.
As an Independent Living Transition Specialist, you will play a crucial role in supporting individuals as they navigate life transitions, overcome barriers, and access vital resources. You’ll be at the forefront of a vital service, helping individuals with disabilities lead more fulfilling, self-directed lives.

Key Responsibilities:
- Empower Individuals: Help adults and seniors with disabilities transition from nursing facilities or other care settings to independent living by providing coaching, peer support, and guidance.
- Collaborate with Community Partners: Build and maintain relationships with local community organizations, healthcare providers, and agencies to secure resources and services needed for successful reintegration.
- Facilitate Person-Centered Planning: Work closely with individuals to develop personalized transition plans that address housing, life skills training, and community support.
- Advocate and Educate: Provide educational support to nursing facility residents and staff about available services and opportunities for independent living.
- Ensure Comprehensive Support: Secure transition funds, provide follow-up services, and monitor progress to ensure individuals’ ongoing success.
- Engage in Continuous Learning: Attend relevant training sessions to enhance your skills and stay up-to-date with best practices in independent living and disability services.
What We’re Looking For:
- Educational Background: A Bachelor’s degree in social work, nursing, or health related field, and/or a Community Health Worker certification is required.
- Experience: At least 3 years of experience in healthcare, disability services, or working with individuals with disabilities is required.
- Commitment to the Mission: Personal experience with disability, or a deep understanding of disability issues and the consumer/community-based independent living philosophy.
- Skills: Strong communication skills, an empathetic and motivational approach, and the ability to connect with individuals from diverse backgrounds.
- Physical Requirements: Ability to sit for extended periods, lift up to 50 pounds, and work in both office and outdoor settings.
- Licensing: Valid Michigan driver’s license and proof of vehicle insurance.

Job Type: Full-time
Pay Rate: $35,000 - $40,000
